Membership Guidelines

• Membership payments are set up as a monthly charge to your credit card.
  

• A credit card or debit card is required for Membership subscription.
  

• There is a $99 non-refundable set-up fee.
  

• Promo codes and monthly specials cannot be combined with Membership discounts.
  

• Must cancel or reschedule an appointment within 48 hrs or will be charged a $100 cancellation fee. 
  

• The first month’s fee is due at the time of Membership subscription purchase.
  

• Each additional month will be charged to your credit card on file on the same day of each month you signed up for.
  

• Cancel anytime with a 90- day written notice period.
  

• For Unlimited members, no retreatment of the same area within 90 days of original treatment.
  

• For skincare members, all treatments must be completed within 14 months.
  

• Toxin credits are non-transferrable.
